Why isn't the break-even price just what I paid for the item?
Because the auction house takes its cut out of the sale price too — the break-even formula divides the buy price by (1 minus the cut percentage) so it also covers the fee taken from that break-even sale.
When does the 15% cut apply instead of 5%?
15% is charged at a neutral auction house — Booty Bay, Gadgetzan, or Everlook — that isn't tied to your faction; posting at your own faction's AH is a flat 5% cut.
